A multi-disciplinary team (MDT) providing care to those with complex and long-term conditions, enabling them to live happier and healthier lives whilst also offering proactive health checks with physician associates.
The Surrey and Sussex Cancer Alliance (SSCA) is working in partnership with virtual health and MHS provider, Medefer, to help identify men with an increased risk for prostate cancer (due to family history of prostate cancer and ethnicity) and offer them a PSA blood test to help expedite diagnosis and treatment, whilst raising awareness of prostate cancer risk factors, signs and symptoms.
This service supports Children and Young People (CYP) and their families with mental health concerns at an earlier stage, helping them to navigate the system, and therefore prevent mental health conditions from escalating.
The team is made up of Health and Wellbeing Coaches, who work holistically with CYPs and families, providing coaching to improve wellbeing, and a CYP Social Prescriber who will carry out goal-based planning and refer to peer groups, sport clubs and art groups where suitable. The team is currently working with CYP aged 4+.

Providing patients with quick and easy access to a range of mental health support and treatment at their local doctor’s surgery. The team will work with the patient to make a plan to get them the help they need and aim to see people within four weeks of being referred. Up to four appointments are usually provided which can be face to face, over the phone or by video call.

We work in partnership with our GP federation Alliance for Better Care CIC to run the Enhanced Access programme which provides out of hours appointments for our patients.
This includes face-to-face and remote appointments on weekday evenings and Saturdays.
